The triliteral root fā yā hamza (ف ي أ) occurs seven times in the Quran, in three derived forms:
three times as the form I verb fāu (فَآءُ)
three times as the form IV verb afāa (أَفَآءَ)
once as the form V verb yatafayya-u (يَتَفَيَّؤُا۟)
