Should الْمُنَادِ not be اسم مرفوع?

You are absolutely right. The expression is actually يناديُ المناديُ but the letter ya in both 50:41:3 and 50:41:4 has been omitted (to facilitate pronunciation).

50:41:3 should be: V – 3rd person masculine singular (form III) imperfect verb, indicative mood (marfu3)

50:41:4 should be: N – nominative masculine (form III) active participle, subject of the verb in 50:41:3
